All physicians and staff of The Private Practice are committed to preserving the integrity and the confidentiality of all protected health information (PHI) pertaining to our patients.
Patients should not be afraid to provide information to our practice and its physicians and staff for purposes of treatment, payment and healthcare operations. To that end, our practice and its physicians and staff will adhere to the standards set forth in the Notice of Privacy Practices
In addition new patients are advised that this practice NEVER releases a full medical record to anyone except the patient! Information from your record will be made available in emergent situations, but insurance companies, etc. will be given only the minimal information they need by way of a form called the "Attending Physicians Statement".
Information that might be vital in future emergencies will be given to the patient to keep at the conclusion of the exam process.
Our practice and physician and staff respect the patient's individual dignity at all times.
Patients should be aware that E-mail can never be totally confidential, and even though it is a very efficient way to convey needed information, set up appointments etc, we will endeavour not to mention specific diagnoses or considerations if possible, consistent with effective communication.
This practice may change this privacy policy in the future. Any changes will be effective upon the release of a revised privacy policy, and will be made available to patients upon request.
